These physical exam cards are the same template I used when entering clinics and needing to keep track of my notes while still getting comfortable with the steps of a veterinary physical exam. Print as many as you want or need, use a digital version on your tablet, or laminate them to use them over and over again.
 
For laminated copies: I recommend using permanent marker to write your patient's exam findings down and then carry them with you all day without it rubbing away (doesn't that sound better than lugging your laptop into the wards?). You can even use the back to take notes on inpatients, take down a history, or make checklists for treatments. When done, use rubbing alcohol and a cloth or paper towel to wipe off your permanent marker and use it again!
